The OHSU Harold Schnitzer Diabetes Health Center (HSDHC) provides diabetes education and disease management to children and adults who have Type 1 and Type 2 diabetes. The HSDHC focuses on patients that have recently been diagnosed with diabetes, those with chronic disease, as well as patients admitted to Doernbecher for new onset diabetes. Care is provided as requested by physicians, other providers, or self-referral to the Center.
The Pediatric Diabetes Educator, working as a member of a multi-disciplinary team, has the primary responsibility for providing diabetes education and medical nutrition therapy for pediatric patients seen at the HSDHC whom have Type1, Type2 and Pre-Diabetes. The Educator will have direct patient contact in the ambulatory and inpatient settings. While in the ambulatory setting he/she/they are responsible for pre-scheduled face-to-face visits with patients and their families/care giver(s) and are responsible for coverage of clinic and telephone triage of patient needs requiring timely response and coordination of care. While in the inpatient setting at Doernbecher Children's Hospital (DCH) he/she/they are responsible for educating children and families/care giver(s) about diabetes survival skills curriculum as well organizing and preparing patients for hospital discharge and follow-up education in the HSDHC. The incumbent is expected to occasionally provide education to the public and community with lectures, talks, cooking classes and other engagements invited to participate in; work directly with dietetic interns as well as with medical and nursing students during their rotations. The Pediatric Diabetes Educator will participate in Center committees and Center quality improvement and practice enhancement activities.Occasional long hours may be required and evening and weekend work for teaching classes. Physical location: PPV, with classes being held at waterfront OHSU locations on occasion. Position does not maintain a private office. Workspace is shared with other Diabetes Educators in the outpatient clinic.
