The Supervisor is one of two Supervisors in the Community Based Care Program. Both Supervisory Social Workers report up to the Non-Institutional Care Program Manager (Supervisory Social Worker). The incumbent of this position is responsible for all areas pertaining to the Contract Nursing Home (CNH), Community Residential Care (CRC) and the Medical Foster Home programs (MFH). Responsible to the Social Work Chief/designee, practices independently, exercising initiative in day-to-day activities.

Coordinates a variety of activities including outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Medical Center staff and community based service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervision of students as assigned. The incumbent is the Supervisor for any combination of the following programs; Community Residential Care Home (CRCP), and Contract Nursing Home (CNH), Medical Foster Home (MFH), Non Institutional Care (NVCC). Conducts and coordinates surveys of those facilities.
Reviews and approve/disapprove HM/HHA and In-Home Respite services which requires a sound knowledge of the program guidelines and inclusion criteria as outlined in IL 10- 2004-005, Under Secretary for Health's Information Letter Non-Institutional Extended Care.
Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities.
Incumbent establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system. Develops work improvement plans, recommending personnel actions as necessary. Provides advice and counsel to workers related to work and administrative matters. Effects disciplinary measures as appropriate. Approves/disapproves leave requests. Assures that subordinates are trained and fully comply with the provisions of the safety regulations. Responsible for furthering the goals of equal employment opportunity (EEO).
Dealing with the consumer constructively and appropriately. Effectively communicates with and utilizes other disciplines (e.g., nursing, medicine) to facilitate treatment planning and implementation. With few exceptions, participates effectively in team meetings, treatment planning conferences, etc. Collaborates with divergent multidisciplinary team members in a manner that enhances coordination of comprehensive patient care.
A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer.

Their experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. In addition,the candidate must demonstrate the professional Knowledge, Skills and Abilities (KSAs) listed below: 1.Ability to independently organize work, set priorities, and meet multiple deadlines. 2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring. 3.Ability to ensure provision of clinical social work services by supervised social workers. 4.Ability to provide administrative supervision and clinical supervision, including supervision for social work licensure.
