ICL Client Care Manager in Brooklyn, New York

Overview

Position Purpose: Serve as a liaison between clients and clinical staff to ensure that client’s needs are met.

Responsibilities

Essential Functions:

  • Serve as liaison between clients and clinical staff.

  • Monitor the implementation of the care plan to ensure that programs are followed

  • Conduct intake interview, orientation of clients to facility rules and regulations

  • Coordinate schedules with other professional services and care teams

  • Assist clients with adjustment to shelter living with periodic follow-up

  • Motivate the client towards the goal of permanent housing

  • Resolve client inquires and disputes

  • Conduct substance abuse groups and workshops for clients

  • Maintain log book documenting all activities in the shelter on the assigned shift and noting any variance to the normal day to day operation

  • Maintain head count; monitor the census and placement of clients to assure compliance with DHS rules and regulations

  • Plan and organize field trips, house meetings, and special activities for residents and staff

  • Monitor the physical upkeep of the facility

  • Manage and oversee all aspects of the facility in the absence of the Program Director and/ or Social Services Director

  • Oversee Supervise Residential Aides, Maintenance staff, Food Manager, and Nurse’s aides on assigned shifts

  • Performs other duties may be assigned

Qualifications

Educational and Experience Requirements:

Bachelors’ degree, preferably in a human services field and two years related experience. A bachelor’s degree in social work or psychology that included a supervised internship and one year of related experience.
